Mimicry.

Able to take on any form ranging from small to large. Loses this form upon taking an action or being attacked.

Servant of Syzeth.

Cannot be killed while Syzeth is alive and nearby. If killed, it is instead incapacitated and must use its next turn to return to full health and become prone. At the end of each turn, a mirror monster regains all of its health.

Like Syzeth, mirror monsters are weak to the magical powers of the Crystal of Ilus. When within 5 feet of it, the mirror monster’s regenerative powers are temporarily shut down. A mirror monster will actively avoid attacking the holder of the crystal of Ilus unless they are the only reasonable target.

Terrifying Presence.

Upon waking up, a mirror monster and all of its nearby brethren begin howling. Each player must roll as if they have been affected by the terrorize action.

Actions

Multiattack. The Mirror Monster makes two melee attacks and terrorizes a single target.
Rend. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 13 (2d8 + 4) slashing damage.
Terrorize. Melee Spell Attack: +6 to hit, reach 5 ft., one target. Hit: The target is frightened for one minute. At the start of each turn, they may make a DC 13 wisdom save to overcome the fear.

Reactions

Ambush. When a player steps within 15 feet of a hiding mirror monster, the mirror monster may use its reaction to walk up to the player and perform a melee attack.

Mirror Monster

Children of the demon Syzeth. Shape-changing demons, able to take on any large or smaller form. When in combat, they revert to their true forms - Ghastly, pale, hairless humanoids with grotesque long claws.
