Oculatus has advantage on saving throws against being poisoned, and it has resistance to poison damage. It doesn’t need to eat, drink, or breathe, it is immune to disease and it does not need to sleep. Magic can’t put it to sleep.

Immobile.

Oculatus has no limbs, and cannot move except to turn its head. It automatically fails any Strength check or saving throw, except for saves against effects attempting to move it, for which it has advantage. It can use its Strength modifier instead of it’s Spellcasting Modifier for its Excessive Force Hands.

Actions

Multiattack. Oculatus makes as many attacks as it has Hands. It can replace any of its attacks with another Hand ability.
Subduing Hand. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 18 (4d8) force damage. Oculatus can only use this attack if it currently has Force Hands.
Excessive Force Hands.

While it has no other Force Hands active, Oculatus creates two Large hands of shimmering, translucent force in two unoccupied spaces that it can see within 60 feet. The hands last for the spell’s duration, and each moves at Oculatus’s command.

Each hand is an object that has AC 20 and 33 hit points. If it drops to 0 hit points, that hand dissipates. It has a Strength of 26 (+8) and a Dexterity of 10 (+0). The hand doesn’t fill its space. It has a fly speed of 60 feet.

Grasping Hand.

Oculatus’s hand attempts to grapple a Huge or smaller creature within 5 feet of it. The hand makes a check using its Strength contested by the Strength (Athletics) or Dexterity (Acrobatics) check of the target. If the target is Medium or smaller, it has advantage on the check. If the target fails, it is grappled by the Hand. While the hand is grappling the target, Oculatus can use a bonus action to have the hand crush it causing the target to take 10 (2d6 + 3) bludgeoning damage.

Interposing Hand.

The hand acts as a wall, barring passage through a space Oculatus chooses. The hand remains in place until given a different command. The hand can only bar entry from one direction at a time. If used as a barrier against attacks against Oculatus, the hand provides it with half cover against the attacker. Creatures can’t move through the hand’s space in the direction it is protecting unless its Strength score is greater than the hand’s Strength score, in which case the square is treated as difficult terrain.

Shoving Hand.

While the Warden has at least one of its Force Hands, it can have the hand attempt to push a creature within 5 feet of it in a direction Oculatus chooses. The hand makes a check using its Strength contested by the Strength (Athletics) check of the target. If the target is Medium or smaller, the hand has advantage on the check. If the target fails, the hand pushes it up to 20 feet. The hand moves with the target to remain within 5 feet of it.

Legendary Actions

The Oculatus can take 3 legendary actions choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The Oculatus regains spent legendary actions at the start of its turn.
Backhand. If Oculatus has at least one Force Hand, it moves one of its hands and uses one of the Hand actions.
Cellblock Tango. An empty cell opens, and Oculatus attempts to use Shoving Hand to push a character inside. If successful, the cell closes and locks.
Lair Action: Antilife Shell. The room Oculatus is in becomes protected by an Antilife Shell, as per the spell, stopping at the doorway. Living creatures within that area are not pushed out, but cannot re-enter if they leave the area.
Lair Action: Antimagic Field. A 30-foot radius cylinder antimagic field centered on Oculatus envelopes the room from floor to ceiling until the beginning of its next turn. The Force Hands are treated as summoned creatures by this effect and wink out of existence while the field is present. Additionally, using this ability ends any antilife shells that Oculatus had previously activated.
Jailbreak (Costs 2 Actions). Release one creature to become hostile, offering a reduced sentence. The creature enters initiative and may or may not be hostile.
