Segmented Body.

When the Rotworm takes more than 15 damage on a single turn, one of its body segments explode. Each creature on a 10 ft. radius must succeed on a DC 15 Dexterity saving throw or take 10 (4d4) necrotic damage or half as much on a successful save.

Actions

Multiattack. The Rotworm makes three attacks: one with its bite and two with its scales.
Bite. Melee Weapon Attack: +10 to hit, reach 5 ft., one target. Hit: 17 (2d10 + 6) piercing damage plus 5 (1d10) necrotic damage.
Scales. Ranged Weapon Attack: +10 to hit, range 30/60 ft., one target. Hit: 8 (1d10 + 3) necrotic damage.

Reactions

Unending Life. When a Rotworm that is large or larger takes 15 damage or more in one turn, it splits into two more Rotworms if it has at least 20 hit points. Each new Rotworm has hit points equal to half the original Rotworm’s rounded up. New Rotworms are one size smaller than the original Rotworm.

Legendary Actions

The rotworm can take 3 legendary actions choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The rotworm regains spent legendary actions at the start of its turn.
Attack. The Rotworm makes a bite or scales attack.
Horrific Stare. Each creature of the Rotworm’s choice that is within 120 feet of the Rotworm and aware of it must make a DC 13 Wisdom saving throw or become frightened for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success. If the creature’s saving throw is successful or the effect ends for it, the creature is immune to the Rotworm’s Horrific Stare for the next 24 hours.
Move. The Rotworm moves up to half its speed without provoking opportunity attacks.
