Heartless Captain.

Captain Soriano Cavanaugh’s heart is bound within a bottle, which in turn is enshrined within Captain Soriano Cavanaugh’s ship. While its heart is protected this way, Captain Soriano Cavanaugh has the same hp as its ship at all times unless it is on a different plane of existence from the ship, and feels any damage inflicted to its ship. Captain Soriano Cavanaugh is otherwise immune to all damage. If the ship is reduced to 0 hp, or if the bottled heart is destroyed, Captain Soriano Cavanaugh instantly dies. If Captain Soriano Cavanaugh is killed while on another plane (or through some other means), it rejuvenates in the captain’s cabin of the ship after 1d6 days, unless its heart or ship have been destroyed.

Multiattack. Captain Soriano Cavanaugh makes 4 attacks. It can choose between throwing a dagger or attacking with its Rapier of Wounding for each attack.
Rapier of Wounding. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 8 (1d8 + 4) piercing damage. Hit points lost to this weapon’s damage can be regained only through a short or long rest, rather than by regeneration, magic, or any other means. Once per turn, when Captain Soriano Cavanaugh hits a creature with an attack using this magic weapon, it can wound the target. At the start of each of the wounded creature’s turns, it takes 1d4 necrotic damage for each time Captain Soriano Cavanaugh has wounded it, and it can then make a DC 15 Constitution saving throw, ending the effect of all such wounds on itself on a success. Alternatively, the wounded creature, or a creature within 5 feet of it, can use an action to make a DC 15 Wisdom (Medicine) check, ending the effect of such wounds on it on a success.
Dagger. Melee or Ranged Weapon Attack: +7 to hit, reach 5 ft. or range 20/60 ft., one target. Hit: 6 (1d4 + 4) piercing damage. Captain Soriano Cavanaugh carries 6 daggers total.
Rapier. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 8 (1d8 + 4) piercing damage.

Reactions

Parry. The captain adds 2 to its AC against one melee attack that would hit it. To do so, the captain must see the attacker and be wielding a melee weapon.
Riposte. If a creature misses Captain Soriano Cavanaugh with a melee attack, it can make an Opportunity attack against that creature. To do so, the captain must see the attacker and be wielding a melee weapon.
